Moog Subsequent 37 MIDI CCs & NRPNs

The Subsequent 37 has parameters that are controllable by both CC and NRPN messages. The relative divisions of the values for each message type are not always related. For instance, a parameter might accept CC values of 0-63 for 'off', and 64-127 for 'on', and NRPN values of 0 for 'off' and 1 for 'on'. Because the MIDI Guide format currently only supports one usage field per parameter, and the CC and NRPN usages are not compatible, this definition has separated the affected parameters into separate rows for CC and NRPN messages.
